Eligibility & Details
- Intended for qualifying first-time Colorado in-state driving offenses and certain Colorado DMV matters involving driver's license revocation or reinstatement requirements.
- Out-of-state offenses, repeat or more involved matters, and referrals requiring detailed clinical documentation use the Standard Substance Use Evaluation.
- Report timing begins after the evaluation and required information have been received.
- Acceptance requirements are determined by the requesting party.
Standard Substance Use Evaluation
Expanded multi-page substance use assessment with multiple assessment tools, requirement review, records review when needed, and expedited report options.

Private pay, superbills, report timing & revisions
Private pay: Colorado Assessments does not bill health insurance or Health First Colorado (Medicaid). A detailed paid invoice or superbill is available upon request for clients who wish to seek possible reimbursement. Reimbursement is determined by the insurance plan and is not guaranteed.
Report timing: Timelines begin after the clinical evaluation is complete and all required records, forms, testing, collateral information, or other requested materials have been received.
Report revisions: After a final report has been issued, client or third-party requested revisions, additions, or amendments may be subject to a $150 fee per revision request. Corrections to typographical, administrative, or factual errors made by Colorado Assessments are completed at no charge.
Requested changes remain subject to clinical judgment. Payment of a revision fee does not guarantee a change to clinical findings, diagnoses, recommendations, or professional opinions.

Pricing & Report Timing
Clinical Mental Health Evaluations start at $597 for a standard-scope evaluation completed within the available clinical and credential scope.
Standard report timing is within 10 business days after the evaluation is completed and all required information has been received.
An expedited Same-Day / Next-Day report may be available starting at $797+, based on availability, clinical appropriateness, case complexity, documentation requirements, and receipt of required materials.
More extensive evaluation requirements, additional sessions, specialized review, or referral to another credential level may result in different pricing.
Credential Requirements Before Scheduling
Clients should confirm the required provider credential with the court, probation department, employer, attorney, agency, licensing body, or other requesting party before scheduling.
Some referrals specifically require a licensed psychologist, psychiatrist, physician, doctoral-level evaluator, or another designated credential or specialty.
If a requirement falls outside the available credential or service scope, Colorado Assessments will not represent its evaluation as satisfying that requirement.
Referral options may be discussed in those cases. Referral availability, evaluation scope, and pricing are discussed at that time based on the required provider and the client's specific needs.
Clinical Scope & Limitations
This is a clinical mental health evaluation.
It is not presented as a psychological or neuropsychological evaluation, competency evaluation, criminal-responsibility evaluation, PRE, CFI, child-custody evaluation, psychosexual evaluation, psychiatric medication evaluation, or another specialty evaluation requiring a different credential or scope.
Evaluation scope and third-party acceptance depend on the specific referral question and requesting-party requirements.
Complex Legal Matter Evaluation
Individually scoped substance use, clinical mental health, and legal matter evaluation services for higher-complexity, attorney-involved, or justice-involved cases.
Base Evaluation, Additional Services & Pricing
The base Complex Legal Matter Evaluation starts at $2,887 and requires a minimum of three clinical sessions.
The starting price reflects the base evaluation scope agreed to during intake. It is not an all-inclusive legal, consultation, or expert-witness fee.
Additional separately priced services may include extra clinical sessions, extensive records review, records retrieval, third-party or collateral interviews, third-party information gathering, attorney consultation, case preparation, expert witness consultation, deposition participation, testimony, court appearance, travel time, travel-related expenses, expedited work, or other case-specific professional services.
The expected scope and applicable fees are reviewed during intake. Additional fees are disclosed and agreed to before the related additional work is performed.
If the scope changes after services begin, additional services and fees are discussed before that additional work is undertaken.
Attorney & Expert Witness Support
When appropriate to the agreed scope, Colorado Assessments may communicate or consult with the client's attorney, review attorney-provided materials, discuss clinical findings with counsel, and assist with clinically relevant case preparation.
Expert witness consultation, deposition participation, testimony, or court appearance may also be available when clinically and professionally appropriate.
Attorney consultation and expert-witness services are separate professional services unless expressly included in the written scope. Availability, anticipated time, travel requirements, and applicable pricing are discussed during intake or before the additional service is accepted.
Whether Adam Krolicki is accepted or qualified as an expert witness in a specific proceeding is determined by the applicable court or tribunal.
Complex Evaluation Scope & Limitations
Attorney involvement alone does not automatically require a Complex Legal Matter Evaluation.
This service does not replace a CFI, PRE, child-custody evaluation, psychological or neuropsychological evaluation, competency assessment, domestic-violence risk assessment, parenting-time recommendation, or legal opinion.
Documentation & Accommodation Support

Standard Documentation & Accommodation Support is $297.
Standard documentation or report timing is within 10 business days after the clinical assessment is completed and all required forms and supporting information have been received.
Documentation, recommendations, forms, and letters are based on the clinical assessment and professional judgment. Payment does not guarantee that a requested letter, recommendation, diagnosis, or accommodation will be clinically supported.
The employer, school, housing provider, licensing body, organization, or other requesting party establishes its own documentation, credential, and acceptance requirements.
Colorado Assessments cannot guarantee that documentation will be accepted by a particular organization or third party.
Housing Assistance Animals & ESA Documentation
Housing assistance-animal or Emotional Support Animal documentation is considered only when clinically supported and legally appropriate.
For Colorado housing assistance-animal requests, applicable Colorado requirements include an in-person clinical meeting, sufficient familiarity with the client and disability, and professional qualification to make the relevant determination.
Documentation may be provided only when the clinical findings support the required disability and disability-related need determinations within the clinician's lawful professional scope.
Colorado Assessments does not provide airline or air-travel Emotional Support Animal letters or certifications.
Colorado Assessments does not train, certify, or register service animals.
Scope, Credential & Acceptance Requirements
Employers, schools, testing organizations, housing providers, licensing bodies, and other institutions may require specific forms, provider credentials, testing, diagnoses, or documentation standards.
Clients should provide the organization's written requirements or forms before scheduling, whenever available.
If the requested documentation requires a credential, specialty, testing process, or evaluation outside the available scope, Colorado Assessments will not represent its documentation as meeting that requirement. Referral options may be discussed when available.
Third-party approval or acceptance is not guaranteed.
